You are responsible for providing accurate billing and payment information. Any insurance, reimbursement, health savings account, or flexible spending account eligibility is determined by the applicable plan administrator and is not guaranteed by Invigorate Chiropractic.

9. Cancellations, Rescheduling, and Refunds

Appointment cancellations, missed appointments, and rescheduling requests are governed by the cancellation policy communicated to you when scheduling or included in your patient or financial agreement.

Late cancellations and missed appointments may be subject to a fee of $50.00

Fees for services that have already been provided are generally nonrefundable. Refunds for prepaid services, programs, events, products, or other purchases will be handled according to the applicable agreement or policy provided at the time of purchase.

Please contact our office as soon as possible if you need to cancel or reschedule an appointment or have a concern about a charge.
